them to live independently and with dignity in their own homes. We
make house calls - enabling access to quality wrap-around services
for homebound older adults with complex health needs while
improving the quality of their lives, right at home.We offer an
array of medical and chronic care services, and we're backed by
SCAN, a not-for-profit organization that has a history of
supporting and improving the health and independence of older
adults for over 45 years.We are currently searching for an Advanced
Practice Clinician (Nurse Practitioner or Physician Assistant) to
join our expanding house call practice. In this highly autonomous
role, you will be responsible for primary care in the home setting,
Televideo or partner clinics in the San Jose, CA area. Our house
call practice embraces a collaborative style approach that allows
you to practice medicine the way you envisioned: patient
centered!We are looking for a full-time provider ideally 40 hours
per week - weekdays to see patients in clinic, in the home, and
Televideo.Responsibilities IncludeVietnamese language ability
strongly preferred.
- Perform primary care in the home setting, via Televideo or in
partner clinics.
- Drive to patient locations in your assigned geographic area in
San Jose.
- Average 7-10 patient visits per day.
- Exhibit stellar bedside manner with the ability to communicate
complex medical information with simplicity and kindness.
- Provide health education to patients. Work with patients in the
prevention of illness, promotion of health, and management of
patient's adjustment to chronic health problems.
- Provide referrals to appropriate medical resources.
- Utilize our EMR system to document patient visit/interaction
accurately and timely.
- Knowledge of HCC coding.
- Active participation in an on-call rotation is required.

- Other duties as assigned.Qualifications And Experience
- Graduate of an accredited Physician Assistant or Nurse
Practitioner program required.
- Current, unrestricted CA license to practice in California
including DEA license.
- Minimum of 2 years of experience as an Advanced Practice
Provider in primary care, hospice, palliative or urgent
care/outpatient setting.
- Bilingual Vietnamese strongly preferred.
- Previous experience in a home health, hospice or other home
visit setting is highly desired.
- Minimum of 1 year of previous experience utilizing an EMR
system with a good working knowledge of HCC coding
- Strong computer skills including MS Office and Outlook is
strongly desired.
- Great verbal and written communication skills
- Current drivers' license, auto insurance and reliable
transportation
