Physical Therapist Location: San Jose, CA Agency: Lancesoft Inc. Pay: $2,908 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position We are seeking a dedicated Physical Therapist to join our home health team. In this role, you will be responsible for assessing, planning, organizing, and participating in rehabilitative programs aimed at improving patient mobility, relieving pain, increasing strength, and improving or correcting disabling conditions resulting from disease or injury. You will build strong relationships with patients, their families, and other healthcare providers through competent patient assessment, effective communication, and attentive monitoring and care. Position Highlights: Shift: 5x8 Days (08:00 AM – 05:00 PM) Weekend Requirements: Rotating weekends (every 6-7 weeks) Key Responsibilities: Patient Assessment & Planning: Assess patient needs and develop personalized care plans to improve mobility, strength, and reduce pain. Rehabilitative Programs: Implement and monitor rehabilitative exercises and interventions that aim to improve or correct mobility impairments caused by disease or injury. Effective Communication: Collaborate with patients, their families, and other healthcare providers to gain cooperation and ensure effective rehabilitation outcomes. Ongoing Care & Monitoring: Provide continuous, attentive care and monitor patient progress, adjusting plans as necessary.
