Must-Have: - Epic Experience - Knowledge of post-acute levels such
as Home Health, Hospice, Palliative Care, Skilled Nursing Facility,
Long-Term Acute Care, Behavioral Health, Sub-Acute, and Acute
rehabilitation Description: The RN Case Manager is responsible for
Care Coordination, Care Transitions, and Discharge Planning
throughout the acute care patient experience. The role involves
completing timely initial assessments, updated notes, and final
notes. The RN Case Manager will complete referrals to post-acute
levels of care. Coordinating with the multidisciplinary team to
ensure the discharge is on track is a key responsibility.
Experience in patient ratio management and understanding admission
criteria is beneficial. Knowledge of CMS guidelines and HIPAA
regulations is required. The position may require floating to other
